Referral

Once we have received a referral from your doctor, we will contact you within 3-5 business days to schedule an evaluation.

Initial Evaluation

Based off of your medical history and current concerns, a comprehensive assessment will be completed to establish your strengths and challenges.

Plan with Purpose
